Exercise your rights under the DPDPA 2023

Use this page to exercise your rights under the Digital Personal Data Protection Act, 2023 (DPDPA) in respect of personal data held by Altx Convergence Private Limited (altx forms / app.altxforms.ai). We acknowledge every request within 48 hours and respond substantively within 30 days of receiving a complete request, in line with our published Privacy Policy.

If you are a respondent to a form built on altx forms — please contact the account-holder (form-owner) first. They are the Data Fiduciary for the response data they collected. If you cannot identify or reach the form-owner, write to dpo@altx.one and we will assist.

1. YOUR RIGHTS UNDER THE DPDPA 2023

  • Right of Access (Section 11) — obtain a summary of the personal data we hold about you and the processing activities concerning it.
  • Right to Correction and Erasure (Section 12) — correct inaccurate or incomplete data; request erasure where the purpose is no longer being served, subject to legal-hold exceptions (e.g., billing records retained under the Income Tax Act).
  • Right to Grievance Redressal (Section 13) — raise any grievance about our processing of your personal data.
  • Right to Nominate (Section 14) — nominate another individual to exercise your rights on your behalf in the event of death or incapacity.
  • Right to Withdraw Consent (Section 6(4)) — withdraw consent at any time, prospectively. Withdrawal does not affect prior lawful processing.
  • DPBI Escalation (Rule 14(iii)) — if you are not satisfied with our response, you may approach the Data Protection Board of India after 30 days (absolute maximum 90 days). Portal: https://dpboard.gov.in (to be enabled by the Board).

2. SUBMIT A REQUEST

The fastest path is via our authenticated app: sign in at app.altxforms.ai and use Account Settings → Data & Privacy → Data Rights. The form pre-fills your registered details and reaches the DPO instantly.

If you do not have a signed-in account, use the form below or write directly to dpo@altx.one. Include enough detail for us to identify your record. We may need to verify your identity by replying to your registered email or asking a corroborating account-context question (e.g., the form on which you submitted a response, the approximate date).

3. WHAT HAPPENS AFTER YOU SUBMIT

  1. Acknowledgement — within 48 hours. You receive an email with a Reference Number (DPR-YYYY-XXXXXX).
  2. Identity verification — within 5 business days. We may need to verify your identity before sharing or modifying data. We do this proportionately.
  3. Substantive response — within 30 days. For erasure, this is the time to initiate; completion timeline follows the retention rules for the relevant data category.
  4. Escalation, if needed. Every response carries a pointer to the Data Protection Board of India for escalation under Section 13.
